# How clear aligners work

Clear aligners move teeth through a planned sequence of custom removable trays\. Successive trays apply pressure for the movements in a person's treatment plan\. The tray is the main appliance, but it is only one part of care: an assessment establishes the problem and goals, progress reviews check the movement, and retention has a different job after active treatment\.

### Why treatment uses a sequence of trays

An aligner is a thin tray formed to fit an individual's teeth\. Active treatment uses a series rather than one unchanged tray\. The trays apply pressure in sequence to carry out the planned tooth movements\. The sequence is central to the mechanism: treatment progresses through appliances intended for the planned movement\.

Think of the sequence as the appliance part of a larger plan\. The clinician determines the approach from the assessment and intended goals\. The number of trays and the treatment period depend on the movements involved\. This is why a useful explanation connects the sequence to the person's treatment goals\.

A sequence describes the appliance progression; the goal explains its purpose\.

### Trays, attachments and other tools have different jobs

Some aligner plans include attachments: small tooth\-colored additions placed on teeth to help the aligners move them\. An attachment is not another tray\. It is a component associated with the planned movement\. The AAO also describes aligners being used with orthodontic elastics in some approaches\.

The useful distinction is function\. A removable tray, a tooth\-attached component and an elastic can all appear in an aligner discussion without being interchangeable\. A product name alone does not establish which components belong in an individual's proposal\.

Read a component name by the job it describes

| Component | General function | Plan\-specific detail |
| --- | --- | --- |
| Active aligner tray | Applies pressure within a sequence of planned movements | The person's movements, sequence and instructions |
| Attachment | A tooth\-colored addition that helps an aligner move a tooth | Whether it is proposed and its individual placement |
| Orthodontic elastic | An additional tool that may be used within an approach | Whether it is part of this plan and its instructions |
| Retainer | Holds finished tooth positions after active treatment | The design and directions for the retention phase |

Identify each component by the job it does\.

### The treatment goal organizes the appliance

The appliance describes a way to apply force\. The treatment plan explains what the clinician intends to address and why that approach is proposed\. Those are separate pieces of information\. 'Clear aligners' answers a category question; it does not explain the assessment findings, the intended bite or alignment changes, or the limits of a proposal\.

A useful explanation connects those pieces in order: what was found, what treatment is intended to change, how the proposed appliance fits that goal, and how progress will be reviewed\. For example, 'aligners' names an appliance, while 'address how the upper and lower teeth meet' describes a possible treatment goal\.

The appliance and the intended treatment goal are different parts of the explanation\.

### Progress review checks more than the tray number

A sequence describes the intended appliance progression\. A progress review checks how treatment is actually developing\. The AAO describes check\-ups during aligner treatment to assess tooth movement and address issues\. The number printed on a tray does not replace that clinical review\.

Review also gives the clinician context about the actual appliances\. The AAO emphasizes accurate information when an aligner does not fit as expected or use has been difficult\. Those observations help the clinician interpret progress, rather than treating advancement through tray numbers as the only thing that matters\.

Progress review connects intended movement with what is happening during treatment\.

### Active aligners and retainers can look similar but serve different purposes

An active aligner belongs to a sequence intended to move teeth\. A retainer is used after active treatment to hold finished positions\. Some retainers are clear removable trays, so appearance alone is a poor way to tell which job an appliance has\. The distinction is its purpose within the plan\.

Retention therefore belongs in the treatment conversation before a person treats the active sequence as the entire proposal\. The transition should identify the retention appliance and its own directions\. Both the active sequence and the retaining appliance belong in an explanation of the complete treatment journey\.

Similar\-looking clear appliances can have different purposes\.

### Worked example: removable does not mean tray\-only

Imagine a fictional conversation in which someone hears 'removable aligners' and then learns that tooth\-colored attachments may be part of the proposed approach\. Those descriptions are compatible\. 'Removable' describes the tray\. An attachment is an addition placed on a tooth to help the aligner move it, so the tray can be removable while a movement aid remains on the tooth\.

This distinction explains why an aligner discussion may involve more than trays\. Some approaches also use orthodontic elastics\. Understanding the different components makes the arrangement clearer: the main appliance is removable, while other tools can have their own places and purposes within treatment\.

Removable describes the tray, rather than every possible component of treatment\.
