# Understand your aligner treatment plan

An aligner plan explains more than which trays will be used\. It connects assessment findings to intended goals, an appliance approach, individual directions, progress review and retention\. Reading those parts in order helps you understand the clinician's explanation before turning to the separate question of what the written fee estimate includes\.

### Start with the findings and the intended goals

Assessment findings describe what the clinician has identified\. Treatment goals describe what the proposed care is intended to address\. The AAO explains that orthodontic assessment considers bite relationships and supporting information as well as visible tooth alignment\.

The distinction matters when a person's first concern is a particular tooth or the appearance of a smile\. That concern is a useful starting point, but the clinician may need to explain how it relates to a broader bite or alignment finding\. A useful plan makes the connection clear rather than leaving an appliance name to carry the explanation\.

Before the assessment, note your main concern in ordinary language and the practical topics you want explained\. Use the homepage planner to organize those priorities, then use the visit page for the Miramar consultation request\.

Connect what was found with what treatment is intended to address\.

### Connect the proposed approach to its directions

The approach identifies the appliance and its purpose within the plan\. For aligners, that includes a sequence of custom trays\. Some approaches also use attachments or elastics\. The presence of a brand name alone does not explain these components or why they are proposed\.

Individual directions describe how the person is to use the actual appliances\. Keep those directions with the named approach\. A general description of aligner therapy explains the mechanism; the prescribed routine and changes belong to the person's clinical plan\.

Keep the named approach and its individual directions together\.

- [Read directions and progress\-review information separately](</aligner-wear-and-follow-up>)

### Understand what review and a proposed change are for

Progress review connects the intended plan with what is happening during treatment\. The AAO describes check\-ups to assess movement and address issues, and emphasizes accurate communication about appliance use and difficulties\. A planned sequence and a clinical review therefore describe different parts of care\.

If the team proposes a change, separate its clinical purpose from its written service scope\. The clinical explanation concerns what the change is intended to address\. The scope explanation concerns the services or charges described in the proposal\. Keeping both explanations makes a later discussion easier to follow without assuming that every possible change is included\.

A proposed change has a clinical purpose and may also raise a written\-scope question\.

- [Use included, separate and not\-confirmed labels for service scope](</understand-your-aligner-estimate#included-separate-not-confirmed>)

### Include the transition to retention

Active treatment moves teeth toward the planned positions\. Retention has the different purpose of holding finished positions after active treatment\. A proposal is easier to understand when it explains both stages instead of ending the conversation at the last active tray\.

Identify the retention appliance being discussed, its directions and the stated follow\-up arrangements\. Then read the estimate for what it says about those services\. The appliance's purpose and the financial inclusion are different questions\.

The holding stage belongs in the complete treatment explanation\.

### Worked example: annotate the parts of a proposal

Imagine a fictional explanation with five headings: findings, goal, appliance approach, review and retention\. Reading it by purpose makes it easier to see what each part contributes\. The goal describes an intended change; the appliance describes a tool for carrying out the approach; review describes oversight; retention describes holding after active treatment\.

A note such as 'aligner system discussed' fills the appliance category, but it does not fill the findings or goal categories\. A separate note about an estimated payment fills a financial category\. The exercise is to connect the explanations, not to judge a clinical plan from the presence of a heading\.

Fictional annotated outline, not a clinic treatment protocol

| Heading | What the explanation contributes | Keep separate from |
| --- | --- | --- |
| Findings | What the clinician identified | The person's original preference |
| Goal | What the proposal is intended to address | The appliance's product name |
| Approach | The proposed appliance and its purpose | The written payment arrangement |
| Review | How treatment progress is assessed and discussed | A tray number alone |
| Retention | Holding finished positions after active treatment | Active tooth movement |

Read each heading for its purpose in the overall explanation\.

### Keep the clinical plan and the estimate connected but distinct

The plan explains the intended care\. The estimate explains the stated service scope and payment arrangement\. Read them together while keeping their jobs distinct: understanding the clinical goal does not settle the charges, and a monthly figure does not explain the clinical goal\. Use the detailed estimate guide for the financial comparison\. Use the visit page to find the studio and request an in\-person discussion\.
