Understanding an assessment finding

# Understanding an interarch tooth\-size finding

A tooth\-size finding can describe a relationship between the upper and lower teeth\. Reading it means keeping three things together: which teeth were compared, which arch supplies the reporting reference and what the relative finding says\.

If your clinician has explained a Bolton tooth\-size finding while discussing orthodontic or aligner care, this companion helps you follow that language\. Start with the relationship, then read the completed example below\.

Adapted from Abd Rahman, Othman and Marya’s 2023 study\. [Read the source context and full credit](<https://clearalignersmiramar.com/understanding-an-interarch-tooth-size-finding#article-credit>) \.

## Tooth width and a two\-arch proportion answer different questions

Interarch means between the upper and lower dental arches\. In a tooth\-size analysis, crown width means the side\-to\-side width of a tooth’s crown\. The widths of the teeth in a defined set are added together in each arch, and those upper and lower totals are compared as a proportion\.

An individual width describes one tooth\. A summed width describes a selected set in one arch\. The interarch proportion describes how the two sets relate\. Larger absolute crown widths do not, by themselves, establish a larger upper/lower tooth\-size proportion\. That is why looking at one tooth, or calling teeth generally large, does not supply this two\-arch finding\.

## The label tells you which teeth belong in the comparison

An anterior comparison uses the six front teeth in each arch\. An overall comparison uses a broader set, from the first molar on one side to the first molar on the other side in each arch\. Each label therefore identifies its own upper set and matching lower set\.

Keep that scope with the finding\. An anterior finding describes the front\-tooth comparison; its wording alone does not supply an overall finding\. The set label answers “which teeth?” before the reporting reference answers “relative to what?”

## A completed reading: set, reference and relative finding

This teaching example starts with three supplied items from an already established finding\. It is a language example, with no patient measurements or invented clinical record\.

**Compared set**

**Anterior comparison\.** The summed crown widths of the six upper front teeth are compared with those of the six lower front teeth\.

**Supplied reporting reference**

**Lower arch\.** The lower front\-tooth set supplies the reference for expressing this finding\.

**Received finding**

**Upper relative excess\.** The finding describes the upper front\-tooth set in relation to that supplied lower reference\.

**Read together:** this is a finding about the upper front\-tooth set relative to the supplied lower front\-tooth reference\. “Upper relative excess” places the described excess on the upper side of that named comparison\. The anterior label keeps the statement about the two front\-tooth sets; the supplied lower reference explains the direction of its wording\.

A reporting reference is the basis used to express the comparison\. It is not a declaration that the lower arch is clinically normal\. Likewise, this relative finding does not establish that the raw upper total is greater than the raw lower total, identify one abnormal upper tooth or select which arch to change\. It does not establish the presence or absence of a separate finding\.

The useful interpretation is therefore complete at the relationship level: you know the compared sets, the supplied reference and which set the relative wording describes\. Individual tooth findings, goals and care decisions still need the responsible clinician’s explanation\.

## Keep the finding connected to the individual plan

A reported proportion is one assessment finding\. Its wording does not choose aligners, a particular system or a procedure\. The study’s authors also identify uncertainty about which deviations affect final treatment outcomes; the finding alone does not predict your result\.

The next useful connection is to the goals and steps your clinician explains\. [Read how assessment findings, treatment goals and review fit an aligner proposal](</understand-your-aligner-plan>) \.

The source is a clinic\-based Malaysian pretreatment comparative cross\-sectional cast study in Kuala Lumpur, not a treatment trial or guideline\. Methods reports 150 patients and 300 casts, while the abstract says 150 study casts; this guide retains that difference without reconciling it\. The study used complete erupted permanent dentition from first molar to opposite first molar and good pretreatment casts\. It excluded width\-affecting restorations or caries, congenital or deformed teeth, obvious wear, prior or ongoing orthodontic treatment, and mixed ancestry as classified by the study\. An age range was not established in the retained narrative\.

The authors did not establish a Malaysian national norm\. This guide adopts no US norm, personal threshold, ethnic or sex\-based prediction, treatment recommendation or outcome forecast\.
